3. Appointment & Cancellation Policy

We ask that patients provide at least 24 hours' notice when canceling or rescheduling an appointment. This allows us to offer that time slot to other patients who may need care.

• Patients who fail to arrive for a scheduled appointment without notice will be recorded as a "no-show."
• Repeated no-shows (three or more) may result in dismissal from the practice.
• Late arrivals of more than 15 minutes may need to be rescheduled.

5. Payment & Insurance

Co-payments, deductibles, and coinsurance amounts are due at the time of your visit. We accept the following insurance plans:

• Blue Cross Blue Shield (BCBS)

• UnitedHealthcare
• Medicare
• Medicaid

Self-pay options and payment plans are available for uninsured patients. Please contact our billing department for details. Patients are responsible for any balance not covered by their insurance plan.

6. Telehealth Services

Dallas Primary Care offers telehealth services subject to the following conditions:

• Informed consent for telehealth services is required prior to your first virtual visit.
• Patients must have access to a device with a camera, microphone, and stable internet connection.
• Telehealth has inherent limitations — certain conditions may require an in-person evaluation.
• The same standard of care applies to telehealth visits as in-person visits.
• Telehealth services are subject to the same billing and insurance policies as in-office visits.
